How to Convert XLSForm to PDF (Paper Forms)

To convert an XLSForm to PDF, you generate a printable paper version of your digital survey. Today you can do this with a form's print view, a paper-form generator, or by laying it out manually — and a one-click tool, FormPrint, is on the way. If you are new to the format first, start with What Is XLSForm?.

A PDF of your survey is more than a nicety. It's a paper backup when devices fail, a reference sheet for enumerators, an artifact for ethics-board review, and an archival copy of exactly what you asked.

Why convert an XLSForm to PDF?

  • Paper backup — collect on paper when a device dies or the battery runs out.
  • Enumerator reference — a printed copy helps interviewers follow long or complex forms.
  • Low-connectivity sites — some field contexts still require paper alongside digital.
  • Ethics and approval — review boards and partners often want the full questionnaire as a document.
  • Archival — keep a permanent record of the exact instrument used.

How to convert an XLSForm to PDF today

There are three practical methods right now. None is perfect, but each works.

Method 1: Print view from Enketo

ODK Central and KoboToolbox render forms with Enketo, which has a print option. Open the form, choose print, and "print to PDF" from your browser. This is the fastest method and preserves question text and choices. If you still need a Central instance, the ODK hosting guide walks through self-hosted and managed options.

Method 2: A paper-form generator

Community tools (such as the PMA "PPP" / ODK printer projects) convert an XLSForm into a formatted HTML or PDF questionnaire from the spreadsheet. These can produce cleaner layouts but often need setup and may not support every question type.

Method 3: Manual layout in Word

For full control of formatting, recreate the questionnaire in Word or Google Docs from your XLSForm and export to PDF. Slow, but it gives you a polished, branded document.

Warning:

Paper can't execute skip logic, calculations, or constraints. When converting any XLSForm to PDF, add manual routing instructions ("If No, skip to Q12") and print all answer options, or the paper version won't match the digital flow.

The limits of current methods

Every method above shares the same gaps:

  • Skip logic and constraints don't translate to paper automatically.
  • Layout control is limited (print view) or labor-intensive (manual).
  • Multi-language forms usually need a separate pass per language.
  • Repeats and grids rarely print cleanly.

FormPrint: one-click XLSForm to PDF (in development)

We're building FormPrint to turn any XLSForm into a clean, print-ready PDF questionnaire in one click — with skip-logic routing notes, every choice listed, and multi-language support handled for you. FormPrint is not released yet, and the methods above are the way to do this today.

In the meantime, you can build, validate, and manage the underlying XLSForm with LooprAI — describe your survey in plain English and it generates an ODK-ready form. See How to Build an XLSForm with AI.

"Paper isn't dead in field research — it's the backup that saves a survey day when the tablets don't cooperate. Getting a clean PDF out of your XLSForm should be one click."

Frequently Asked Questions

How do I convert an XLSForm to PDF?

Open the form in a renderer like Enketo and use its print option to print to PDF, use a paper-form generator that converts the spreadsheet to a formatted document, or recreate the questionnaire in Word and export to PDF. Each preserves questions but not skip logic.

Can I print an XLSForm as a paper questionnaire?

Yes. You can produce a paper questionnaire from an XLSForm using a form's print view, a paper-form generator, or a manual layout. Add manual routing instructions because paper cannot run the skip logic and constraints a digital form enforces.

Does converting XLSForm to PDF keep the skip logic?

No. PDF and paper are static, so skip logic, calculations, and constraints do not carry over automatically. When converting, add written routing notes such as 'If No, skip to Q12' and print every answer option so the paper form matches the digital flow.

Why create a paper backup of a digital survey?

Paper backups keep fieldwork running when devices fail or batteries die, give enumerators a reference for complex forms, satisfy ethics-board and partner requirements, and provide a permanent archival record of the exact instrument used.

Is there a one-click XLSForm to PDF tool?

SurveyLoopr is building FormPrint to convert any XLSForm to a print-ready PDF in one click, including skip-logic notes and multiple languages. It is in development and not yet released; today you can use a print view, a paper-form generator, or a manual layout.
